Refund, Transfer, and Donate Form
To receive a refund, please complete the attached form and the W-9 form. Return the form to the LCM Child Nutrition Department. Refunds may take up to six weeks.
To transfer funds or donate funds to another sibling or student, please complete the first page of the attached form and return the form to the LCM Child Nutrition Department.
